Many people assume that once you’re discharged from the hospital, you’re ready to go home. But more and more, patients are being referred to rehabilitation facilities (skilled nursing) to get the therapy they need to live safely and comfortably at home.

So who makes the decision where you’ll be going after you’re released from the hospital? The Discharge Planner is responsible for ensuring that patients get the care they need after a surgery, illness or injury. Some patients can get this care at home. Others need further therapy and recuperation. In those cases, the Discharge Planner will provide you with a list of rehabilitation facilities from which to choose – a decision you’ll have to make with little time to research and make an informed decision. That’s where the RAPID RECOVERY SUITES card would help to streamline this process.

If you or your loved one is in the hospital, you already have enough on your mind.
